Care Manager, Golden Care, Foundation of the Greater Miami Jewish Federation

Location: Miami-Dade County

The Greater Miami Jewish Federation seeks a talented care manager professional for Golden Care, an innovative program of The Foundation of the Greater Miami Jewish Federation.

This position reports to the Golden Care Director.

Areas of Responsibility:

    The successful candidate will work with the Director of Golden Care and Foundation Director to provide support, advocacy, and referral services for philanthropic seniors who can afford their personal expenses, but who may not have family or other resources available to provide the assistance they need to remain independent.
  • Maintain weekly contact with clients
  • Advocate for clients at medical appointments.
  • Provide referrals for doctors, living arrangements, community services, and home healthcare
  • Assist with pharmacy issues
  • Assist with any insurance issues, to include Long Term Care policies, Medicare or secondary insurance
  • Communicate with family members as necessary
  • Arrange medical appointments for clients
  • Drive patients to all appointments
  • Plan opportunities for social engagement such as luncheons or holidays
  • Attend appropriate Greater Miami Jewish Federation meetings
  • Document and maintain all relevant client medical information and interactions in the electronic database records
  • Participate in the Golden Care Committee meetings
  • Plan and execute programs to market Golden Care in the community
  • Job Requirements

    A college degree or certification in geriatric case management, demonstrated written and oral communication skills, and outstanding interpersonal skills are required. Knowledge of Jewish culture and the organized Jewish community is a plus along with an understanding of and commitment to the Federation’s mission. The ideal candidate will have a high energy level and be proactive and results-oriented. He/she must be entrepreneurial and flexible and demonstrate the ability to work collegially with others at all levels of the institution and the community. He/she will also have a warm personality, ability to listen and set boundaries, sensitivity to the needs and problems of the elderly, strong knowledge of community resources available for referrals, and the ability to make difficult decisions, especially during crises. Salary is commensurate with skills and experience.
