Greater Miami Jewish Federation Travel Policies on Insurance

I acknowledge that the Greater Miami Jewish Federation does not currently, and will not in the future, provide me with any medical insurance coverage while on the Mission. I understand that any medical insurance coverage I desire must be purchased for the Mission on my own.

I further acknowledge that GMJF does not provide trip cancellation insurance for Mission participants and any coverage that I desire must be purchased for the Mission on my own.

GMJF strongly recommends that all Mission participants review their own personal insurance coverage needs with their insurance advisors to determine if additional travel insurance is required. Additional travel insurance could cover items such as reimbursement for trip delay, missed connections, sickness and/or accident medical expenses, lost baggage, personal effects, baggage delay, etc. Each participant must make their own decision regarding insurance, as the Greater Miami Jewish Federation is not authorized to give insurance advice.

Greater Miami Jewish Federation Missions- Israel and Worldwide Release of Liability for Death, Personal Injury and Property Damage

I am aware of the risks of travel to, from and within Israel and countries worldwide including risks associated with my safety and security. These risks include, but are not limited to, property damage and loss, death and injury by accident, disease, or terrorist act. I am voluntarily participating in the mission to Israel (“the Mission”) sponsored by the Greater Miami Jewish Federation (“GMJF”) and/or one or more of its members (“GMJF”) with a full understanding of these risks, and I assume and agree to accept those and all other risks to my safety and security during the course of my participation in the Mission.

I have read, or have had the opportunity to read, the current United States Department of State’s Travel Advisory for Israel, the West Bank and Gaza. I understand that, in advance of the Mission, I may periodically check the State Department’s website, found at https://www.travel.state.gov, to see if the Travel Advisory has been superseded by a new Travel Advisory.

I have read, or have had the opportunity to read, the current United States Department of State’s Worldwide Caution. I understand that, in advance of the Mission, I may periodically check the State Department’s website, found at http://www.travel.state.gov, to see if the Worldwide Caution has been superseded by a new Worldwide Caution.

I acknowledge and affirm that, notwithstanding any security arrangements that may be made by GMJF, GMJF does not, and cannot, guarantee and is not responsible for the safety of my person or property during the Mission or any Mission-related activities, including, but not limited to, airline travel, ground transportation, meals, lodging, recreational activities or “free time” activities.

In light of the above alerts and circumstances and in consideration of my being permitted to participate in the Mission, I do, for myself, my spouse, heirs, executors, administrators and assigns, release and forever discharge GMJF, as well as its respective subsidiaries, members, affiliates, predecessors, successors and assigns, and all of their respective past, present and future officers, directors, shareholders, members, employees, agents, and contractors, and their respective heirs, executors, administrators and assigns (each singularly, a “Releasee” and, collectively, the “Releasees”), of and from any and every claim against any Releasee or Releasees, arising from or by reason of any bodily injury, personal injuries (including emotional trauma), death, or property damage resulting or alleged to result from any accident, incident, or other episode or cause, whether known or unknown, that may occur during or as a result of my participation in the Mission, whether based upon the negligence of, or breach of contract by, any Releasee or any other party for whose acts or omissions any Releasee or Releasees may be responsible under any applicable legal principle.

This release contains the entire agreement between me and GMJF and supersedes any prior or contemporaneous agreements, understandings, and negotiations regarding the Mission. This release shall be interpreted and enforced in accordance with the laws of the State of Florida without regard to its conflict of law rules, and shall be construed as broadly and inclusively as permitted by such laws. If any provision of this release is held invalid by any court of competent jurisdiction, in such event, the balance of this release shall continue in full force and effect and be enforced as if the invalid portion were not contained in this release.

I have carefully read this release, understand its contents and acknowledge that this is a release of liability and such is a binding and fully enforceable contract between me and anyone traveling with me as my guest under this registration including any minors, on one hand, and the GMJF and all Releasees on the other hand.

Having consulted, or having had the opportunity to consult, my own counsel as to the meaning and legal effect of this release, I have voluntarily signed this release on the date noted below.

The Greater Miami Jewish Federation abides by all security regulations put in place by Israel’s Ministries of Tourism and Education (via the “Moked-Teva Situation Room), which coordinates with the Israel Defense Forces, Israel Police, and other security authorities.

I have carefully read the foregoing release, understand its contents and acknowledge that this is a release of liability and such is a binding and fully enforceable contract between me, on one hand, and the GMJF and all Releasees, on the other hand.

Israel, the West Bank and Gaza Travel Advisory

Do not travel to the West Bank due to COVID-19. Exercise increased caution when traveling to the West Bank due to terrorism and civil unrest. Do not travel to Gaza due to COVID-19, terrorism, civil unrest, and armed conflict.

The Centers for Disease Control and Prevention (CDC) has issued a Level 3 Travel Health Notice for Israel due to COVID-19, indicating a high level of COVID-19 in the country. Your risk of contracting COVID-19 and developing severe symptoms may be lower if you are fully vaccinated with an FDA authorized vaccine. Before planning any international travel, please review the CDC's specific recommendations for vaccinated and unvaccinated travelers.

Terrorist groups and lone-wolf terrorists continue plotting possible attacks in Israel, the West Bank, and Gaza. Terrorists may attack with little or no warning, targeting tourist locations, transportation hubs, markets/shopping malls, and local government facilities. Violence can occur in Israel, the West Bank, and Gaza without warning. There has been a marked increase in demonstrations throughout Israel, some with little or no warning.

West Bank: U.S. government travel throughout the West Bank is extremely limited. Visit the Department of State's website for Travel to High Risk Areas.

Gaza: The U.S. government is unable to provide emergency services to U.S. citizens in Gaza as U.S. government employees are prohibited from traveling there and are restricted from traveling close to the Gaza demarcation line. Hamas, a U.S. government-designated foreign terrorist organization, controls the security infrastructure in Gaza. The security environment within Gaza and on its borders is dangerous and volatile. Sporadic mortar or rocket fire and corresponding Israeli military responses may occur at any time. During periods of unrest or armed conflict, the crossings between Gaza with Israel and Egypt may be closed.

If you decide to travel to Israel and/or the West Bank:

  • See the U.S. Embassy's COVID-19 page.
  • Visit the CDC’s webpage on Travel and COVID-19.
  • Have a plan to depart Israel and the West Bank, which does not rely on U.S. government assistance.
  • Carry additional supplies of necessary medication in carry-on luggage due to quarantine restrictions.
  • Check the most recent Alerts at the Embassy website for the latest information on travel in all of these areas.
  • Maintain a high degree of situational awareness and exercise caution at all times, especially at checkpoints and other areas with a significant presence of security forces.
  • Avoid demonstrations and crowds.
  • Follow the instructions of security and emergency response officials.
  • Beware of and report suspicious activities, including unattended items, to local police.
  • Learn the location of the nearest bomb shelter or other hardened shelter. Download the Home Front Command Red Alert application for mobile devices to receive real time alerts for rocket attacks.
  • Obtain comprehensive travel medical insurance that includes medical evacuation prior to travel. Most travel insurance packages do not cover mental health related illnesses/care.
  • Enroll in the Smart Traveler Enrollment Program (STEP) to receive Alerts and make it easier to locate you in an emergency.
  • Follow the Department of State on Facebook and Twitter.
  • Review the Overseas Security Advisory Council (OSAC) Crime and Safety Report for Israel, the West Bank, and Gaza.
  • U.S. citizens who travel abroad should always have a contingency plan for emergency situations. Review the Traveler’s Checklist.

Terms and Conditions Applicable to All Mission Travel

The traveler(s) signing and submitting this application understand and agree that the following terms and conditions shall apply to all portions of the mission travel contemplated hereby (the “Mission”), and that the following provisions shall run in favor of, and, where applicable, be enforceable by, Greater Miami Jewish Federation, Inc., a Florida not-for-profit corporation (together with its officers, directors, employees, agents, representatives, successors and assigns, as applicable [collectively, the “Federation”]).

Your participation in the Mission is completely voluntary and optional, and, if you do not wish to be bound by these terms and conditions, you should not complete this form and you should not participate in the Mission.

The Federation provides absolutely no insurance protection of any kind relating to your participation in the Mission. You are strongly encouraged to privately and independently secure any and all desired insurance coverage concerning, without limitation, trip disruption/cancellation/rescheduling; loss of property (including jewelry or other valuables); health insurance; accident insurance; life insurance; or any other insurance of any nature whatsoever.

During the Mission, you will be required to strictly adhere to all local, state, federal and international policies and directives concerning COVID-19 and other viruses and pandemics of any type (including the known and unknown variants of all of them), including those concerning testing, social distancing, quarantining, the wearing of face coverings, and other similar public health requirements, at your expense.

The laws and regulations of local, state, federal and international governments may require you to quarantine in a foreign location, at your expense, without physical access to family and friends, and could result in the interruption or cancellation of the Mission.

Except as indicated on the payment schedule, all amounts paid by you for the Mission are non-refundable, even if the Mission is interrupted, rescheduled or canceled for any reason whatsoever.

The Mission involves potential risks of illness, infection, disease, injury, paralysis, disfigurement and/or death, whether on account of influenza, viruses, pandemics of all types (including, without limitation, COVID-19, and the known and unknown variants of all of them), accidents, weather events, natural disasters, insect/animal encounters, food/water impurities, criminal acts, violence, civil unrest, armed conflict, terrorist attacks, war, bombs, explosions, acts of G-D or other foreseeable or unforeseeable eventualities of any kind (collectively, the “Risks”). You voluntarily and expressly assume the risk of each and all of the Risks, and separately covenant that (a) you are solely responsible for your personal safety and welfare during the entirety of your participation in the Mission; and (b) the Federation is not and will not be liable under any theory of recovery for any physical injuries, impairments, losses, costs or damages of any nature whatsoever suffered or incurred by you during, or later arising from, directly or indirectly, your participation in the Mission.

By participating in the Mission, you irrevocably agree to (a) permanently waive, release, surrender and discharge any and all claims of any nature whatsoever (including, without limitation, any demands for attorneys’ fees and costs) that you may have against the Federation that directly or indirectly relate to or arise from at any time your participation in the Mission (the “Claims”); and (b) indemnify and hold the Federation harmless from the Claims, and any potential or actual liabilities, costs and expenses (including attorneys’ fees) flowing therefrom.

In consideration of your participation in the Mission, you irrevocably agree never to sue or otherwise assert against the Federation in any court, arbitration proceeding, administrative proceeding, agency, board, panel or other tribunal anywhere in the world any legal or equitable claim of any nature whatsoever, under any theory of recovery, for any injuries, impairments, losses, claims, costs or damages that directly or indirectly relate to or arise from your participation in the Mission.
